If you are in our practice area then please call at the surgery to collect a New Patient Questionnaire or click here to download.

You can also now register online using the link below.

You will need to complete this questionnaire and bring it to the surgery in person. Richmond Medical Centre is a Safe Surgery and your nationality or immigration status do not affect your right to register here. If you find it difficult to communicate, you can ask our reception for an interpreter who can help support you with the registration process. We might ask you for ID or proof of address, but if you don’t have any and you live in our practice area, you can still register with us.

Once registered with the practice you can make an appointment to see any of the doctors or nurses.  You can express a preference to receive services from a particular doctor or nurse, although some have longer waiting times than others.

Please inform us of any change in address, telephone number, email or name. This will enable us to contact you quickly should we need to.

Out of Area- Practice Policy

As part of our commitment to providing safe, effective, and accessible healthcare, we are only able to care for patients who reside within our designated practice boundary.

What This Means for You

If you move to an address outside of our catchment area, we will no longer be able to keep you registered with the practice. This is to ensure we can continue to offer timely home visits (if needed), appropriate referrals, and care that complies with NHS guidelines.

What You Need to Do

  • If you are planning to move or have recently moved out of our practice area, please let us know as soon as possible.
  • You will need to register with a GP practice that covers your new address. You can find a suitable GP by visiting the NHS Find a GP service.
  • We will continue to provide care until you are accepted by a new practice, but this will be for a limited time only.

Catchment Area (Practice Boundary)

Named GP

A new Government scheme now requires every patient to be allocated a Named GP who is responsible for patients’ overall care at the practice. The allocation of a Named GP for ALL Patients has now been completed. If you would like to find out which GP you have been allocated, please enquire the next time you are in the surgery.

As a patient, you will still be able to book an appointment with any GP at the surgery. You also have the right to request a change to your allocated GP if you have a preferred choice GP. The practice will make reasonable efforts to accommodate this request.
